The Hyperhidrosis Disease Severity Scale (HDSS) is a validated, single-question, 4-point rating that turns the subjective experience of excessive sweating into a number a clinician or trial can track. It was developed and validated by Kowalski and colleagues (presented at the 2004 American Academy of Dermatology meeting) and has since become the most widely used severity endpoint in hyperhidrosis research. A score of 3 or 4 defines severe hyperhidrosis; a 1-point improvement after treatment corresponds to roughly a 50% reduction in gravimetrically measured sweat, and a 2-point improvement to roughly an 80% reduction. What the HDSS actually is

The HDSS is a single item. The patient reads four statements about how sweating interferes with daily activities and picks the one that best fits: (1) never noticeable and never interferes, (2) tolerable but sometimes interferes, (3) barely tolerable and frequently interferes, or (4) intolerable and always interferes. The chosen number is the score.

That simplicity is deliberate. The instrument was designed to be answered in seconds, without training aids, in either written or interview form, so that it could be used in a routine clinic visit and repeated to gauge treatment response. Scores of 1 or 2 are described as mild-to-moderate disease; scores of 3 or 4 are described as severe.

How HDSS improvements map to measured sweat

The most clinically useful thing about the HDSS is that it was anchored to objective sweat measurement. In the validation work, improvements in HDSS after treatment tracked closely with reductions in gravimetrically measured axillary sweat.

The validation evidence

In Kowalski's original two studies, HDSS scores at 4 weeks after treatment correlated with the Dermatology Life Quality Index (DLQI) and the Hyperhidrosis Impact Questionnaire (HHIQ), with reported correlations of r = 0.35 to 0.77 (p < 0.001). Test-retest reliability was r = 0.82. Higher HDSS scores were associated with greater self-reported interference across daily activities in a national survey, supporting the scale as a measure of impact rather than just wetness.

The instrument's responsiveness (its ability to move when a real treatment effect happens) was demonstrated by pairing HDSS change with gravimetric change: patients whose HDSS improved by 1 point had, on average, about half as much measured sweat, and those improving by 2 points had about a fifth of their pretreatment sweat. This is the anchor that turned the HDSS from an opinion poll into a trial-grade endpoint.

How the HDSS is used in clinical trials

In modern hyperhidrosis trials, a baseline HDSS of 3 or 4 is the standard inclusion criterion, because it defines the population with clinically meaningful disease. The most common primary or co-primary endpoints are (a) achieving an HDSS of 1 or 2 after treatment, (b) at least a 2-point HDSS improvement from baseline, and (c) at least a 50% reduction in gravimetric sweat. Trials of onabotulinumtoxinA, topical glycopyrronium, and topical anticholinergic wipes all use variants of these thresholds.

Regulatory reviews likewise treat a 2-point HDSS improvement (or a total HDSS score of 2 or less) as clinically meaningful. This is the operational answer to "did the treatment work?" in most published research.

What the HDSS does not do

The HDSS is powerful for its simplicity, but that simplicity has trade-offs:

  • It is a single question. It captures overall interference with daily life, not location, frequency, or amount of sweat.
  • It was validated primarily in primary axillary hyperhidrosis. Its behavior in craniofacial, plantar, or generalized secondary hyperhidrosis is less well characterized, and other scales (e.g. HDSM-Ax, ASDD) are used alongside it.
  • It is subjective. It is meant to complement, not replace, objective measures such as gravimetry when quantification of sweat volume matters.
  • It is coarse: a patient can genuinely improve without crossing a full point. Clinical trials therefore usually combine HDSS with a percent-reduction gravimetric endpoint.

Methodology and limitations

The scale was validated in primary axillary hyperhidrosis and is best supported in that population.
