What perimenopause is, and how long it lasts
Perimenopause is the stretch of time when the ovaries wind down and menstrual cycles become irregular, ending 12 months after the final menstrual period (the point that defines menopause itself). According to the U.S. Office on Women's Health, perimenopause can last between two and eight years before periods stop permanently, and for most women this transition lasts about four years. The average age of menopause in the U.S. is 52, with the usual range running from about 45 to 58.
Because "perimenopause" covers several years of gradual change, researchers needed a common vocabulary to describe where a woman is in the process. That is what the STRAW+10 system provides.
The STRAW+10 staging system
The Stages of Reproductive Aging Workshop + 10 (STRAW+10), published in 2012 and led by Siobán Harlow, is the reference standard for staging reproductive aging. It anchors everything to the final menstrual period (FMP), labeled stage 0, and defines stages before and after it using menstrual-cycle patterns supported by hormone measures such as FSH.
The stages run from the late reproductive years (Stage -3), through the early and late menopausal transition (Stages -2 and -1, which together make up perimenopause), across the final menstrual period, and into early and late postmenopause (Stages +1 and +2). STRAW+10 is designed to apply regardless of a woman's age, ethnicity, body size, or lifestyle.

Durations are those STRAW+10 specifies; Stages -3 and -2 are not assigned a fixed length. FSH thresholds are approximate and support, rather than replace, the menstrual criteria.
When symptoms are most likely
Hot flashes and night sweats do not appear uniformly across the transition. STRAW+10 notes that vasomotor symptoms are most likely during the late menopausal transition (Stage -1) and early postmenopause. SWAN data show that once frequent hot flashes begin, they last far longer than the old "a few years" assumption: a median of 7.4 years in total, and about 4.5 years continuing after the final period.

How much timelines vary between women
The stages give structure, but individual experience varies widely. In SWAN's trajectory analysis of hot flashes across the transition, women fell into roughly four groups: about 27% had a consistently low chance of symptoms, about 26% had a persistently high chance throughout, around 18% had early-onset symptoms that began well before the final period, and about 29% had late-onset symptoms that rose sharply just after it. In other words, no single timeline fits everyone: some women have years of symptoms before their last period, others mainly afterward, and a substantial minority have few at all.
Symptoms other than hot flashes (such as sleep disruption, mood changes, and cycle irregularity) also cluster around the late transition and early postmenopause but follow their own patterns. Persistent, disruptive, or unusual symptoms (for example, very heavy bleeding or bleeding after menopause) are worth discussing with a clinician, both for symptom relief and to rule out unrelated causes.
