Sauna Use, Sweating, and Health Research

A sauna is one of the most reliable ways to make the body sweat heavily, and Finnish research has linked regular sauna use to better cardiovascular outcomes. In the Kuopio Ischaemic Heart Disease (KIHD) study, which followed 2,315 middle-aged men for a median of 20.7 years, men who used a sauna 4 to 7 times a week had substantially lower cardiovascular and all-cause mortality than men who used one once a week (Laukkanen 2015). A typical session produces roughly half a litre of sweat and raises heart rate to levels seen in moderate exercise (Hannuksela and Ellahham 2001). This page traces each figure to its source. What a sauna does to the body

In a hot, dry Finnish sauna (typically 80 to 100°C at head height), skin temperature climbs quickly and the body mounts a strong heat-loss response. Blood vessels in the skin widen, heart rate rises to roughly 100 to 150 beats per minute, and the eccrine sweat glands pour out fluid to cool the skin by evaporation. A single session commonly produces on the order of half a litre of sweat, which is why hydration afterward matters (Hannuksela and Ellahham 2001).

The cardiovascular load during passive heating resembles low-to-moderate physical exercise: cardiac output increases and blood is redistributed toward the skin. This overlap is one reason researchers became interested in whether regular sauna use might carry some of the same associations with health that regular activity does. The sweating itself is the body cooling down; it is not, by itself, a treatment or a detox.

The cardiovascular association: the KIHD study

The most cited evidence comes from the Kuopio Ischaemic Heart Disease Risk Factor Study, a prospective cohort of 2,315 middle-aged men (aged 42 to 60) from eastern Finland, followed for a median of 20.7 years. Participants were grouped by how often they used a sauna each week. After adjusting for cardiovascular risk factors, more frequent sauna use was associated with progressively lower risk of sudden cardiac death, fatal coronary heart disease, fatal cardiovascular disease, and death from any cause (Laukkanen 2015).

Compared with men using a sauna once a week, those using one 4 to 7 times a week had a hazard ratio of 0.37 for sudden cardiac death (about 63% lower) and 0.60 for all-cause mortality (about 40% lower). Longer sessions (over 19 minutes) were also associated with lower risk than short ones. A later KIHD analysis that included women reported a similar cardiovascular mortality signal (Laukkanen 2018).

KIHD outcomes by sauna frequency

Hazard ratios compare men using a sauna 2 to 3 times a week, or 4 to 7 times a week, against men using one once a week, after adjustment for cardiovascular risk factors. A hazard ratio below 1.0 indicates lower risk.

These are observational associations, adjusted for measured risk factors, in a specific population of Finnish men. They cannot by themselves prove that sauna use causes the lower risk.

Why these findings need caution

The KIHD results are striking, but several features of the research limit how far they can be pushed:

  • The core findings are observational. People who use saunas very frequently may differ in health, wealth, activity, or social connection in ways that also lower mortality, so association is not the same as cause.
  • The main cohort was middle-aged Finnish men in a culture where sauna use is routine, which limits how directly the numbers transfer to other populations and settings.
  • Sauna bathing was self-reported at a single baseline, and habits can change over a 20-year follow-up.
  • The benefits, if causal, are most plausibly linked to the cardiovascular effects of passive heating rather than to sweating removing anything harmful from the body.

Sweating, hydration, and safety

It is worth separating the sweating from the health signal. Sweat is mostly water with some electrolytes; its job in a sauna is evaporative cooling, not clearing toxins. The fluid lost as sweat is replaced by drinking afterward, and the transient drop in body weight after a sauna is water, not fat. Claims that sweating in a sauna detoxifies the body are not supported by the physiology.

Saunas are generally well tolerated by healthy people, but the heat load is real. Dehydration, dizziness on standing, and reduced tolerance are more likely with long sessions, alcohol, or in people who are unwell. Anyone with heart disease, low blood pressure, who is pregnant, or who takes medications affecting fluid balance or blood pressure should treat sauna use as a question for their own clinician rather than assume the cohort findings apply to them.

How to read this evidence

The honest summary is that regular sauna use is associated with lower cardiovascular and all-cause mortality in large Finnish cohorts, with a dose-response pattern by frequency and session length. That is a meaningful and repeatedly reported signal, but it comes from observational data and cannot on its own establish that sauna use causes the benefit. The heavy sweating is a marker of the heat exposure, not the mechanism of any benefit.

This page describes what the studies found, not a recommendation to start or increase sauna use. Whether sauna bathing is appropriate for a given person depends on their health and medications, which is a conversation for a clinician.

Methodology and limitations

This page draws on the Kuopio Ischaemic Heart Disease (KIHD) prospective cohort studies (Laukkanen et al., JAMA Intern Med 2015, n=2,315 men, median 20.7-year follow-up; and Laukkanen et al., BMC Med 2018, which added women), a peer-reviewed review of sauna physiology and risks (Hannuksela and Ellahham, Am J Med 2001), and a narrative review of sauna health evidence (Laukkanen, Laukkanen and Kunutsor, Mayo Clin Proc 2018). Each figure was traced to its source.

Limitations: the mortality findings are observational and adjusted for measured risk factors only, so residual confounding (fitness, wealth, social factors) cannot be excluded, and causation is not established. The main cohort was middle-aged Finnish men, limiting generalizability. Sauna habits were self-reported at baseline. The sweat-volume and heart-rate figures are typical values from physiology reviews, not fixed for every person or sauna. Nothing here is a diagnosis, a recommendation to start sauna use, or medical advice; people with heart or blood-pressure conditions, who are pregnant, or on relevant medications should consult a clinician.
