Aluminum in Antiperspirants: What the Safety Evidence Shows

Two claims circulate widely: that the aluminium in antiperspirants contributes to breast cancer, and that it contributes to Alzheimer's disease. This page reports what the major reviewing bodies have concluded, neutrally and without taking a side for or against using these products. In short, the National Cancer Institute, the American Cancer Society, and a large 2014 systematic review all report that current evidence does not support a clear link between antiperspirant aluminium and either disease. "No clear evidence" is not the same as proof of perfect safety; it means the studies done so far have not found the associations the claims describe. Every figure below is traced to its source.

What the reviewing bodies conclude

The National Cancer Institute states directly that "no scientific evidence links the use of these products to the development of breast cancer." The American Cancer Society reaches a similar position: "there are no strong epidemiologic studies (studies in people) that link breast cancer risk and antiperspirant use, and very little scientific evidence to support this claim."

A 2014 systematic review in Critical Reviews in Toxicology, one of the most comprehensive assessments of aluminium and human health, addressed both concerns in a single sentence: "Neither is there clear evidence to show use of Al-containing underarm antiperspirants or cosmetics increases the risk of AD [Alzheimer's disease] or breast cancer." The same review reported that "metallic Al, its oxides, and common Al salts have not been shown to be either genotoxic or carcinogenic."

These are careful, neutral statements from the sources themselves. They report an absence of demonstrated harm from the claimed mechanisms, not a promotional endorsement of antiperspirants.

Where the breast cancer claim came from

The hypothesis has a plausible-sounding starting point. Antiperspirants are applied frequently to skin near the breast, and the NCI notes that "some research suggests that aluminum-containing underarm antiperspirants... may be absorbed by the skin and have estrogen-like (hormonal) effects." Because estrogen can encourage some breast-cancer cells to grow, the idea followed that aluminium might do the same. It was reinforced by the observation that about half of breast cancers arise in the upper outer part of the breast, nearest the armpit.

The evidence has not borne this out. The American Cancer Society notes that about half of breast cancers develop in the upper outer quadrant "most likely because there is more breast tissue in this area," not because of proximity to antiperspirant. Skin absorption is very low: in one study only about 0.012% of the aluminum applied was absorbed, an amount the ACS describes as much less than what is absorbed from food. A related version of the claim, that antiperspirant blocks the body from "sweating out toxins" through underarm lymph nodes, does not fit the anatomy: the ACS notes that "lymph nodes do not release waste or toxins through sweating" and "are not connected to sweat glands."

Case-control studies back this up. The ACS reports that these studies generally "have not found a link between breast cancer risk and antiperspirant use, deodorant use, or underarm shaving," including a large study that compared roughly 800 women with breast cancer to similar women without it and found no link.

Where the Alzheimer's claim came from

This concern is older and traces to the 1960s, when studies reported unusually high aluminium concentrations in some brains affected by Alzheimer's disease, and animal experiments using very high aluminium doses produced changes in brain tissue. That sparked decades of speculation about aluminium from cookware, cans, and antiperspirants.

Follow-up research has not confirmed the link. The Alzheimer's Society summarizes the position that no study, or group of studies, has been able to confirm that aluminium is involved in the development of Alzheimer's disease, and notes that aluminium in food and drink is in a form that is poorly absorbed, with less than about 1% of what is present taken up, and most of that is cleared by the kidneys. The 2014 systematic review reached the same conclusion for consumer exposure, finding "no consistent and convincing evidence to associate the Al found in food and drinking water at the doses and chemical forms presently consumed by people... with increased risk for Alzheimer's disease."

How to read this evidence

Two points keep the picture honest. First, "no clear evidence of harm" is a statement about what studies have found so far; it is not a claim of proven, absolute safety, and researchers continue to study aluminium exposure. Second, finding aluminium in a tissue sample is not the same as showing it caused a disease; association is not causation, which is exactly why the reviewing bodies weigh controlled studies rather than single correlations. The NCI makes this kind of distinction about parabens too: it notes that although "parabens are found in breast tumors, ... there is no evidence that they cause breast cancer."

This page is not an endorsement of antiperspirants and does not recommend using or avoiding them. Limitations: most human evidence on antiperspirants and breast cancer comes from case-control and cohort studies, which can show association but not prove causation, and none is a randomized trial of antiperspirant use. 'No clear evidence' reflects the current body of research and could change if strong new studies appear; it is not a guarantee of complete safety. The skin-absorption figure (about 0.012%) comes from a single study.
