Palmar Hyperhidrosis (Sweaty Hands): Statistics and Impact

How common are sweaty hands?

In a nationwide questionnaire study in Japan (5,807 valid responses, ages 5-64), primary palmar hyperhidrosis affected 5.33% of respondents. A separate survey of about 13,000 high-school and college students in Fuzhou, China, found primary palmar hyperhidrosis in 4.59% of participants. These two large East Asian samples land close together, in the mid-single-digit-percent range.

Both are prevalence estimates from specific populations (one a broad Japanese sample, the other Chinese adolescents and young adults), so they are best read as "roughly 1 in 20" rather than a single global figure. Palmar hyperhidrosis has been studied especially intensively in East Asia, where much of the surgical and epidemiological literature originates.

Palms in the context of other sweat sites

The same Japanese survey measured primary hyperhidrosis at four body sites. Palms and underarms were the most commonly reported, with soles and head somewhat lower. This puts the palmar figure in context alongside the other focal forms.

Sweaty hands usually start in childhood

Palmar hyperhidrosis characteristically begins early. In a US clinic series of 387 primary hyperhidrosis patients, the mean age of onset for palmoplantar (hands-and-feet) hyperhidrosis was 11.5 ± 8 years, significantly younger than axillary onset (20.0 ± 8.3 years) or craniofacial onset (25.4 ± 13.7 years). The Chinese survey found a similar pattern: the peak age of onset was 6-16 years, accounting for 95.6% of palmar cases.

In other words, sweaty hands are usually a childhood- or teenage-onset trait rather than something that first appears in adulthood, a feature clinicians use to distinguish primary focal hyperhidrosis from later-onset (secondary) causes.

Palmar prevalence across large surveys

Two large studies anchor the palmar prevalence estimate. They used different populations and methods, which is why the headline numbers differ slightly.

Family history and sex

Palmar hyperhidrosis frequently runs in families. In the Fuzhou study, 15.3% of palmar cases reported a positive family history, and the condition affected both sexes roughly equally. A hereditary tendency is consistent with the early, childhood onset seen across studies.

Association with family history is not the same as a fully understood inheritance pattern (reported family-history rates vary by how they are collected), but the clustering is a well-documented feature of primary focal hyperhidrosis.

Impact and low help-seeking

Sweaty palms can interfere with everyday tasks that involve touch and grip (handling paper, tools, phones, or shaking hands) and the social visibility of wet hands is a recurring theme in quality-of-life research on hyperhidrosis. Yet formal help-seeking is strikingly low: in the Japanese survey, only 6.2% of people with focal hyperhidrosis had visited a medical institution about it.

That gap between how common palmar hyperhidrosis is and how rarely it is brought to a clinician suggests many people manage it on their own without knowing it is a recognized, treatable medical condition. Anyone whose hand sweating is distressing, interferes with work or daily life, or has changed suddenly can reasonably discuss it with a clinician, who can also rule out other causes.

Methodology and limitations

This page compiles two large questionnaire-based prevalence surveys (Fujimoto 2013, Japan; Tu 2007, China) and a US dermatology clinic series describing age of onset and body-site distribution (Walling 2009). Each figure was traced to its source abstract and confirmed.

Limitations: prevalence figures come from specific national and regional samples (Japanese general population; Chinese students) and are self-reported via questionnaire, so they are not interchangeable with a single global rate. The onset and body-site figures come from patients already attending a hyperhidrosis clinic, which is not a random population sample. Family history is self-reported.
